Johns Hopkins University School of Nursing Hands off National CAPABLE Program to Care Synergy

Care Synergy is deeply honored to be selected to manage the Community Aging in Place – Advancing Better Living for Elders (CAPABLE) national program. CAPABLE was developed by the Johns Hopkins University School of Nursing to provide older adults with resources to remain in their homes longer, achieve better health outcomes, and decrease medical costs.

 What makes this even more exciting is the Colorado Visiting Nurses Association (CVNA) is the first community-based home health organization in the U.S. to implement CAPABLE. The handoff was official when Dr. Sarah Szanton the Dean of Johns Hopkins University School of Nursing visited the Amy Davis Center and awarded the responsibility of managing the CAPABLE National Center to Tricia Ford, Sr VP of Operations at Care Synergy.

 In addition to overseeing daily operations, Tricia will be responsible for managing Amanda Goodenow, OT Strategic Partnership Coordinator; Colleen Morton, RN CAPABLE Manager; Gina Machado, RN existing sites, expansion of newly licensed sites, discovery efforts to identify funding sources, and administrative support.

 There was no better way to celebrate CAPABLES’s first anniversary when the the Care Synergy team met one of its first milestones of enrolling 100 people into the CAPABLE program.

 Special thanks to Tricia Ford, SVP of Operations at Care Synergy for initiating and managing the relationship with Johns Hopkins as well as heading up the implementation of this strategically important program. In addition, a big thank you to Care Synergy and CVNA employees for taking on this responsibility.
