Care Synergy Provides Mission Support to our Affiliate Organizations
Since its inception in 2014, Care Synergy’s network has grown to four independently operated mission-focused not for profit organizations including The Denver Hospice/Optio Health Services, Pathways, Pikes Peak Hospice and Palliative Care and the Colorado Visiting Nurse Association. This growth allows our network to provide a full continuum of community based care and services.

Colorado Visiting Nurse Association (CVNA) is a nationally recognized leader in the provision of an innovative continuum of care that maximizes individual health, wellness and well-being in the Front Range. CVNA provides the most appropriate range of health care solutions to allow people to maximize independence and to heal and age at home.

Pathways, located in Fort Collins since 1978, provides care for those navigating the last months of life and support programs for their families. Pathways serves individuals living in Larimer and Weld Counties, including Fort Collins, Loveland, Greeley, Windsor and surrounding communities.

Pikes Peak Hospice & Palliative Care
Pikes Peak Hospice & Palliative Care (PPHPC) introduced the hospice movement to the Pikes Peak region in 1980. Since that time, PPHPC teams have continued to offer compassionate, expert care and practical support to ensure comfort for all who face serious illness.
