Pathways Opens Greeley Office to Support Expanded Hospice, Palliative Care and Grief Services in Weld County
GREELEY – Jan. 11, 2021 – Pathways, northern Colorado’s leading provider of hospice, palliative care, and grief and loss counseling services and a Care Synergy affiliate, today opened a new office in Greeley to support its expanded services in Weld County.
Pathways has seen an increase in patients and requests in Weld County in recent years, driving the need for a permanent home in Greeley, in addition to its Fort Collins headquarters. “As the demand for our services increases, we are excited to provide this new home for our staff and community services as we extend our care to more families in Weld County,” said Nate Lamkin, president of Pathways.
Pathways purchased the building at 4075 W. 11th St. and renovated the space throughout 2020. The office includes dedicated areas for grief and loss counseling services and a large community education space where, when it is safe to do so, Pathways staff will offer programs to the community on issues such as advanced illness, end-of-life care, and grief and loss. In addition, the building offers space for Pathways staff who provide care to Weld County patients.
The Pathways headquarters building is at 305 Carpenter Road in Fort Collins. Thanks to the ongoing and generous support of donors, Pathways is building a 12-bed inpatient center adjacent to the office on Carpenter Road.
