National Family Caregivers Month: How Colorado PACE Supports Those Who Care for Others

November is National Family Caregivers Month, a time to recognize the millions of family members and loved ones who dedicate their time, energy and hearts to caring for older adults.

Family caregivers play an essential role in helping aging loved ones maintain their health, independence and quality of life. Whether coordinating medical appointments, managing medications, preparing meals, providing transportation or simply being there when someone needs a helping hand, their contributions are invaluable.

But caregiving can also be challenging. Balancing a loved one's needs with work, family responsibilities and personal well-being can leave caregivers feeling overwhelmed. That's why having a reliable support system matters.

Caregiving Is a Team Effort

At Colorado PACE, we believe family caregivers shouldn't have to navigate the journey alone.

PACE (Program of All-Inclusive Care for the Elderly) provides comprehensive, coordinated care for eligible older adults who meet the program's requirements and wish to remain living in their homes and communities rather than move into a nursing home.

Participants have access to an interdisciplinary team of healthcare professionals who work together to understand their individual needs and develop a personalized care plan.

Services include:

  • Medical care: Primary care and coordination of specialty medical services.
  • Nursing support: Ongoing health monitoring, medication management and clinical guidance.
  • Rehabilitation: Physical, occupational and other therapies to help participants maintain or improve their abilities.
  • Transportation: Rides to medical appointments and the PACE Day Center.
  • Meals and nutrition: Nutritious meals and support with individual dietary needs.
  • Social engagement: Activities and opportunities to connect with others at the PACE Day Center.
  • Care coordination: A dedicated team working together to help participants receive the services and support they need.

Supporting Family Caregivers, Too

These services benefit more than just PACE participants. They also provide meaningful support to the family members and loved ones who help care for them.

Knowing that a loved one has access to a coordinated team of healthcare professionals can offer caregivers reassurance and peace of mind. Transportation, day center activities, medical services and ongoing care coordination can also help ease some of the day-to-day responsibilities that often fall on family members.

Just as importantly, PACE recognizes that family caregivers have needs of their own. Having a team to share the responsibility can create opportunities for caregivers to take a break, attend to personal commitments and focus on their own health and well-being.

Celebrating the People Behind the Care

Family caregivers give so much of themselves to the people they love. Their patience, compassion and dedication deserve recognition and support, not just during November but throughout the year.

At Colorado PACE, we're proud to work alongside family caregivers, helping older adults receive comprehensive care while supporting the people who mean the most to them.

This National Family Caregivers Month, we say thank you to all the family caregivers who make a difference every day. You are not alone, and you deserve a team behind you.
