Colorado PACE Survey Shows 85% Participant Satisfaction with Home Care Visits

Providing high-quality care doesn't stop when a participant leaves a healthcare center. At Colorado PACE, care continues in the places where participants live their daily lives.

Results from the program's first Annual Participant Satisfaction Survey show that 85% of participants reported being satisfied with their home care visits.

Home-based care plays an important role in helping older adults remain safe, independent, and supported while living in their own communities. Through the PACE model, interdisciplinary care teams coordinate services that may include nursing visits, therapy, personal care, and other support delivered directly in participants' homes.

These services ensure that participants receive consistent, high-quality care regardless of location. The same commitment to safety, compassion, and clinical excellence found in Colorado PACE centers extends to home visits throughout the community.

Home care visits also help care teams better understand each participant's environment and daily challenges. By seeing firsthand how participants live, care providers can identify potential risks, recommend safety improvements, and tailor care plans to better support independence.

The strong satisfaction rating in the survey reflects Colorado PACE's commitment to meeting participants where they are—both geographically and personally.

As Colorado PACE continues to expand its services, participant feedback remains a critical tool for maintaining high standards and ensuring care is delivered in ways that truly support aging in place.

Caregiving in Community Event Brings Conversation and Support to Colorado Springs Caregivers

Caregiving is a deeply meaningful role, but it can also bring emotional, physical, and financial challenges. To help spark conversation and provide support for caregivers, Pikes Peak Hospice & Palliative Care partnered with Rocky Mountain PBS and several community agencies to host Caregiving in Community: A Film and Conversation on Support, Strain & Strength this past weekend.

The event brought together caregivers, healthcare professionals, and community members for a film screening followed by a thoughtful discussion about the realities of caregiving.

Honoring the Strength and Sacrifice of Caregivers
Caregivers provide essential support to family members and loved ones facing illness, aging, or disability. Yet many caregivers often feel isolated in their experiences.

Through shared stories and conversation, attendees reflected on the emotional and practical aspects of caregiving while honoring the strength, compassion, and dedication caregivers bring to their role.

Events like this help create space for caregivers to feel seen, heard, and supported.

Strengthening Community Support for Caregivers in Colorado Springs
Community conversations about caregiving play a vital role in building awareness and resources for those providing care. By bringing together local organizations and caregivers, events like Caregiving in Community foster connections that can lead to greater understanding and support.

As a nonprofit hospice and palliative care provider serving the Colorado Springs region, Pikes Peak Hospice & Palliative Care remains committed to supporting caregivers and families throughout the healthcare journey.

We are grateful to our partners, participants, and community members who joined us for this meaningful event. Together, we can continue to strengthen the network of support for caregivers across the region.

Miniature Donkeys Bring Joy to Pathways Inpatient Care Center on National Unicorn Day

Patients and staff at the Pathways Inpatient Care Center experienced a memorable moment of joy and connection during a recent visit from two special guests: Paco and Levi, a pair of miniature donkeys with plenty of personality.

The visit coincided with National Unicorn Day, and the donkeys arrived dressed as unicorns, bringing smiles and laughter to patients, families, and staff throughout the day.

While the costumes added a fun and festive element, the impact of the visit goes deeper. Animal-assisted interactions can play an important role in supportive care settings by helping reduce stress, create moments of happiness, and encourage connection among patients and caregivers.

For individuals receiving serious illness care, small moments of joy can have a meaningful emotional impact. Interactions with animals often spark conversation, evoke memories, and provide a comforting presence during difficult times.

The Power of Community Partnerships
The visit was made possible through a partnership with Hearts & Horses Therapeutic Riding Center, a nonprofit organization that provides equine-assisted services designed to improve well-being and connection.

Collaborations with community organizations like Hearts & Horses help expand the ways care providers can support patients beyond traditional medical services. These partnerships bring new experiences, emotional support, and uplifting moments to those receiving care.

At Pathways, moments like these reflect a broader commitment to whole-person care—addressing not only physical needs but also emotional and social well-being.

Whether through compassionate clinical care or joyful visits from four-legged friends, the goal remains the same: creating meaningful experiences that support comfort, dignity, and quality of life.

Thanks to Paco and Levi, a little extra magic—and many smiles—filled the halls that day.

Colorado PACE Survey Shows 88% of Participants Say Program Meets Expectations

At Colorado PACE, building trust with participants and families begins with delivering on the promises made during enrollment.

Results from the program's first Annual Participant Satisfaction Survey show that 88% of participants reported that Colorado PACE met the expectations set when they enrolled in the program.

Enrollment is often the first step in a participant's journey with Colorado PACE. During this process, participants learn how the program's interdisciplinary care team will support their health needs, independence, and quality of life.

The strong survey results indicate that Colorado PACE is successfully translating those commitments into real-world care and services.

The PACE model brings together physicians, nurses, therapists, social workers, and other specialists to coordinate comprehensive care for older adults who wish to remain living in their communities. By working closely with participants and their families, the care team ensures services align with the goals and expectations established at enrollment.

For participants, meeting expectations means more than simply receiving services—it means feeling confident that the program understands their needs and follows through on its commitments.

As Colorado PACE continues to grow, participant feedback will remain a key part of ensuring the program delivers high-quality, person-centered care that participants can trust.

Colorado PACE Participants Report High Satisfaction in First Annual Statewide Survey

The first-ever Colorado PACE Annual Participant Satisfaction Survey shows overwhelmingly positive feedback from the people who rely on the program every day. According to the survey, 90% of participants said they would recommend Colorado PACE to a friend or family member.

PACE, which stands for Program of All-Inclusive Care for the Elderly, is designed to help older adults with complex health needs remain living safely in their homes and communities. The program provides coordinated medical care, rehabilitation services, social support, transportation, and other essential services through an interdisciplinary care team.

The strong recommendation rate highlights the value participants place on person-centered care, coordinated services, and the ability to age in place while receiving comprehensive support.

PACE programs across Colorado bring together physicians, nurses, therapists, social workers, and other specialists who work closely with each participant to create individualized care plans. This collaborative model allows participants to receive the care they need while maintaining independence and staying connected to their communities.

As Colorado's population continues to age, programs like PACE are becoming increasingly important in providing high-quality healthcare for seniors, reducing unnecessary hospitalizations, and supporting family caregivers.

The results of the first statewide participant satisfaction survey demonstrate the program's impact and reinforce its role as a trusted option for integrated senior care and aging-in-place services in Colorado.

In the west courtyard of the Inpatient Care Unit at The Denver Hospice, a young cherry blossom tree is now in full bloom—offering a beautiful and living tribute to Connie Segura.

The tree was planted last April in Connie's memory, honoring both her life and the compassionate care she received while at The Denver Hospice. Recently, Connie's son Michael returned to the courtyard to visit the tree and capture photographs while its blossoms were at their peak.

During his visit, Michael was joined by Ben Marcantonio, President of The Denver Hospice, and Tricia Ford, Chief Operating Officer of Care Synergy. Together, they reflected on the significance of the memorial tree and the enduring connection families often maintain with hospice communities long after a loved one's passing.

Memorials like Connie's cherry blossom tree serve as powerful reminders that hospice care is not only about providing comfort at the end of life—it is also about honoring lives, supporting families, and creating spaces for remembrance and healing.

Each spring, as the tree blossoms again, it stands as a symbol of love, resilience, and the meaningful relationships formed through hospice care.

For families and staff alike, Connie's tree has become more than a memorial. It is a place of reflection, gratitude, and remembrance—a living tribute that continues to grow, season after season.

Helping older adults remain safely in their homes is one of the most pressing challenges facing communities today. A recent episode of Century Lives: The Home Stretch highlights one promising solution: the CAPABLE Program.

The episode, titled "Capably Aging In Place," features a conversation with three leaders working to advance this innovative approach to healthy aging:

Sarah L. Szanton, Dean and Patricia M. Davidson Health Equity and Social Justice Professor at the Johns Hopkins School of Nursing
Allyson Evelyn-Gustave, CAPABLE Clinician Training Specialist
Shauna Buchmoyer of United Disabilities Services in Lancaster, Pennsylvania

Meeting the Challenges of an Aging Population
By 2030, every Baby Boomer in the United States will be age 65 or older. Many older adults will live alone, often on limited incomes, while managing mobility challenges or chronic health conditions.

At the same time, the nation faces shortages in both housing and senior care. These demographic shifts raise important questions about how communities can support older adults who want to remain in their homes as they age.

Small Changes, Big Impact
The CAPABLE program offers a practical and effective approach. Developed more than 15 years ago at Johns Hopkins, CAPABLE brings together a nurse, an occupational therapist, and a handy worker to help older adults address functional challenges in their homes.

Sometimes the solution is surprisingly simple. Installing grab bars, securing rugs, improving lighting, or adding assistive technology can make the difference between a home that feels unsafe and one that supports independence.

These targeted improvements not only help prevent falls and injuries but also empower participants to manage daily activities more confidently.

Proven Results
Research has shown that CAPABLE can significantly reduce hospitalizations and nursing home stays, while also lowering healthcare costs. As program leaders often say: "Home is where the health is."

With communities across the country exploring new models for aging services, CAPABLE demonstrates how thoughtful, low-cost interventions can make a meaningful difference in the lives of older adults.

Listen to the full podcast episode:
https://open.spotify.com/episode/0CIXd0sqQdzjrTAPnav6kg

Colorado PACE Survey Shows 90% of Participants Understand and Are Satisfied with Their Plan of Care

At Colorado PACE, providing care that reflects each participant's unique needs and goals is central to the organization's mission. Results from the program's first Annual Participant Satisfaction Survey show that this approach is making a meaningful difference.

According to the survey, 90% of Colorado PACE participants reported that they understand and are satisfied with their plan of care.

A clear, personalized plan of care is a cornerstone of the PACE model. Participants work closely with an interdisciplinary care team—including physicians, nurses, social workers, therapists, and other specialists—to develop a plan tailored to their health needs, preferences, and daily lives.

The strong survey results demonstrate the effectiveness of this collaborative approach. When participants understand their care plans and feel confident in them, they are better able to engage in their care and maintain independence.

At Colorado PACE, care planning is not a one-time conversation. Plans are continually reviewed and adjusted as participants' needs change, ensuring care remains responsive and individualized.

These early survey results reinforce Colorado PACE's commitment to meeting participants where they are and partnering with them to support their health, well-being, and quality of life.

As the program continues to grow, participant feedback will remain a critical part of shaping and improving the care experience.

It's been called America's quiet crisis: Caregiving for an aging population.

Last year, 63 million people – nearly one in four adults in the United States – provided ongoing care to a family member, neighbor or friend. According to AARP, among these caregivers, 59 million care for an adult with a complex medical condition or disability, and 4 million care for a child with a medical condition or disability.

Family caregivers are everyday people from all walks of life, transcending age, race, geography or income level. They play vital roles assisting with personal hygiene, managing finances, providing transportation, housekeeping, navigating medical appointments, and providing social and emotional support. A majority of these caregivers are women.

As the population age 85 and older – the group most likely to need assistance – is projected to more than double by 2050, the caregiving crisis will only deepen.

At Pikes Peak Hospice & Palliative Care, it is our privilege to come alongside family members caring for loved ones as they face a life-limiting illness. We are there for the patient – but we are also there for the patient's caregivers, providing support, education, respite, community connections and more. This wrap-around support for both patients and caregivers is why we often hear our families say, "I wish we had turned to Pikes Peak Hospice sooner."

Pikes Peak Hospice is proud to partner with Rocky Mountain PBS to present "Caregiving in Community: A Film and Conversation on Support, Strain & Strength."

The documentary "Caregiving" takes a personal look at the paid and unpaid caregivers navigating the challenges of this meaningful work. It shines a light on the rarely examined difficulties families face as parents and loved ones age. The matter is deeply personal to the film's executive producer, the actor Bradley Cooper, who cared for his father during his battle with cancer.

Following a screening of portions of the film, I'll be among the panel of experts and those with lived experience, providing local context to this important subject and exploring ways our community can support its caregivers.

WHAT: "Caregiving in Community: A Film and Conversation on Support, Strain & Strength," co-presented by Rocky Mountain PBS' Allies for Aging Initiative and Pikes Peak Hospice & Palliative Care
WHEN: 1 p.m. Saturday, April 11. Doors open at 1. Program begins at 1:15 with curated film excerpts followed by a panel discussion and Q&A. From 2:45 to 3:30, guests are invited to peruse community resources.
WHERE: Colorado Springs Senior Center, 1514 N.
Colorado PACE Survey Shows 90% of Participants Feel Encouraged to Be Part of Their Care Decisions

Colorado PACE recently completed its first Annual Participant Satisfaction Survey, and the results reveal strong feedback from the individuals served by the program.

One of the most encouraging findings: 90% of participants reported that they feel encouraged to be involved in their own care decisions.

This feedback reflects a core principle of the Colorado PACE model—ensuring that participants remain active partners in their care planning.

Empowering Participants in Their Care
For many older adults and individuals with complex health needs, maintaining independence and having a voice in care decisions is incredibly important. Colorado PACE is designed to support this by building care plans with participants, not just for them.

Participants work closely with an interdisciplinary care team that listens to their preferences, goals, and concerns. This collaborative approach helps ensure that each care plan reflects the individual's needs, lifestyle, and priorities.

The survey results demonstrate that participants are experiencing this approach firsthand.

Why Participant Feedback Matters
Participant feedback is an important part of improving care. Surveys like this help programs understand what is working well and where opportunities exist to make services even better.

The overwhelmingly positive response from participants reinforces Colorado PACE's commitment to:

	Person-centered care
	Participant engagement
	Collaborative decision-making
Respect for individual preferences and goals

Continuing to Build on Positive Results
While the survey results are encouraging, Colorado PACE views this feedback as part of an ongoing conversation with the people it serves. Listening to participant experiences helps guide improvements and ensures the program continues to meet the needs of the community.

Encouraging individuals to take an active role in their care helps build trust, confidence, and stronger health outcomes.

For Colorado PACE, the message from participants is clear: having a voice in care decisions matters—and it's making a difference.

Colorado PACE Survey Results Highlight Participant Satisfaction and Independence
Colorado PACE recently received the results from its first Annual Participant Satisfaction Survey, offering valuable insight into how the program is supporting older adults across the region.

The feedback was overwhelmingly positive, with participants sharing experiences that demonstrate the impact of coordinated care, personalized support, and comprehensive services designed to help older adults remain safe, healthy, and independent.

One key finding stood out: more than 50% of participants who responded to the survey reported that they are living independently.

Supporting Independence Through the PACE Model of Care
The Program of All-Inclusive Care for the Elderly (PACE) is designed to help older adults maintain independence while receiving the medical care and supportive services they need.

Colorado PACE provides coordinated healthcare, social services, rehabilitation support, and other resources that allow participants to continue living in their homes and communities whenever possible.

The survey results demonstrate that this model is working. By focusing on whole-person care and individualized support, Colorado PACE helps participants maintain autonomy and quality of life.

Listening to Participants and Improving Care
Participant feedback is an important part of ensuring that programs continue to meet the needs of the people they serve. The Annual Participant Satisfaction Survey helps Colorado PACE better understand participants' experiences and identify opportunities to enhance services.

These early results reflect both the strength of the PACE model and the dedication of the care teams who support participants every day.

Helping Older Adults Age in Place
For many older adults, the ability to remain in their own home is closely connected to dignity, comfort, and overall well-being. Programs like Colorado PACE play an important role in making aging in place possible.

By providing coordinated care and supportive services, Colorado PACE empowers participants to maintain independence while receiving the medical and social support they need.

Why Hospice Volunteer Training and Orientation Matter

Volunteering in hospice care is deeply meaningful, but it also requires thoughtful preparation. Hospice volunteers often support people who are facing serious illness, loss, and major life transitions.

That is why Pathways provides a structured volunteer orientation and training program designed to help volunteers feel confident and supported.
During orientation, volunteers learn about:

The philosophy and goals of hospice and palliative care
How hospice volunteers support patients, caregivers, and care teams
Communication skills for meaningful companionship visits
	Patient privacy and confidentiality requirements
Boundaries and best practices when supporting families
Emotional awareness and compassion in end-of-life care

This preparation ensures volunteers are ready to provide compassionate, respectful support while also feeling comfortable in their role.

What Hospice Volunteers Do
Hospice volunteers contribute in many ways depending on their interests, availability, and skills. Their support helps hospice teams focus on providing the highest level of clinical care while ensuring patients receive personal attention and connection.

Common hospice volunteer roles include:

Patient Companionship
Many volunteers spend time visiting with hospice patients. These visits may include conversation, reading aloud, listening to music, or simply providing a comforting presence.

Caregiver Support
Volunteers may provide short periods of respite so family caregivers can rest, run errands, or take care of other responsibilities.

Administrative and Office Support
Some volunteers assist hospice teams with administrative tasks that help programs run smoothly.

Community Outreach and Events
Volunteers may also help with community education, fundraising events, and awareness initiatives that support hospice programs.
Each role contributes to the overall mission of hospice care: supporting quality of life and dignity for patients and families.

The Impact of Hospice Volunteers on Patients and Families
Research and decades of hospice experience have shown that volunteers can have a powerful impact on patient well-being and family support.
For many patients, volunteers provide:

	Social connection and conversation
	Emotional comfort and reassurance
A listening ear and compassionate presence
	Relief from loneliness or isolation

For families and caregivers, volunteers help reduce stress by offering companionship, practical assistance, and emotional support during a difficult time.

These connections are often deeply meaningful for both volunteers and the people they serve.

Hospice Volunteer Opportunities in Northern Colorado
Pathways welcomes volunteers from across Fort Collins, Loveland, Greeley, and surrounding Northern Colorado communities who want to give back in a meaningful way.

People choose to volunteer with hospice for many reasons, including:

A desire to support people during end-of-life care
	Personal experiences with hospice care
A commitment to serving their community
Interest in healthcare, social work, or caregiving fields

No matter the motivation, every volunteer becomes part of a compassionate team dedicated to helping patients live with dignity and comfort.

Becoming a Hospice Volunteer
Becoming a hospice volunteer begins with orientation and training designed to prepare individuals for this unique and meaningful role.

Pathways works closely with volunteers to ensure they feel supported, informed, and ready to serve before they begin working with patients or families.
For those looking for a way to make a lasting difference in their community, hospice volunteering offers an opportunity to provide kindness, presence, and compassion when it matters most.

Learn more about volunteering with Pathways here: https://pathways-care.org/volunteer/

Pikes Peak Hospice & Palliative Care has earned its highest quality ranking ever — a score of 94 out of 100 — on the National Hospice Locator, a nationwide directory that evaluates hospice providers using publicly available quality data.

The National Hospice Locator compiles information from Medicare data files, public websites, and direct surveys of hospice providers. Using multiple weighted quality metrics, the platform evaluates thousands of hospice programs across the United States and allows users to compare providers based on quality rankings.

With a national average score of approximately 67, a score of 94 places Pikes Peak Hospice & Palliative Care well above the national benchmark and among the highest-rated hospice organizations in Colorado.

"This recognition reflects the extraordinary dedication of our clinical teams and support staff who care for patients and families every day," said Dawn Darvalics, President of Pikes Peak Hospice & Palliative Care. "Hospice care is about more than clinical excellence — it's about compassion, presence, and honoring each person's wishes at the end of life. Our team lives those values every day."

For more than four decades, Pikes Peak Hospice & Palliative Care has served the Colorado Springs community, providing compassionate end-of-life care, palliative support, and grief services to patients and families throughout the region.

Quality rankings like those used by the National Hospice Locator incorporate multiple indicators of hospice performance, including data reported to Medicare and other national datasets. These measures help families and healthcare providers identify organizations delivering high-quality care.

"Our mission is to walk alongside patients and families during some of life's most meaningful and difficult moments," Darvalics said. "This recognition reinforces what we strive for every day — care that honors dignity, comfort, and connection."

How the CAPABLE Program Helps Older Adults Age Safely at Home

A Real Story of Aging in Place: Listening to What Older Adults Need
For decades, journalist Paul Pickrem built his career by listening carefully—paying attention not only to what people said, but also to what was left unsaid.
When he participated in the CAPABLE (Community Aging in Place—Advancing Better Living for Elders) program in Nova Scotia, he immediately recognized that same attentiveness.

What stood out most was not simply the home modifications or assistive equipment, but how the CAPABLE team listened closely to his daily challenges and responded with practical solutions.

Paul, who will turn 72 this year, lives with severe arthritis and increasing mobility limitations. Tasks that once felt routine—cutting vegetables, squeezing an eye-drop bottle, or doing laundry—had gradually become difficult.

While traditional medical care addressed diagnoses and treatment plans, it often left the practical realities of everyday life unresolved.
CAPABLE stepped directly into that gap.

By combining nursing support, occupational therapy, and home modifications, the program focuses on what matters most: helping older adults remain safe, independent, and confident in their own homes.

What You'll Find in the Latest CAPABLE Newsletter
The newest CAPABLE newsletter explores the growing impact of this innovative aging-in-place model and highlights key developments in research, policy, and practice.

Real Client Experience
Paul Pickrem's story illustrates how personalized support can transform everyday living for older adults managing chronic conditions and mobility challenges.

Research on Fall Prevention
A recent study published in JAMA Ophthalmology found that the relationship between vision impairment and falls in older adults is strongly influenced by home hazards—reinforcing the importance of CAPABLE's home safety approach.

Evidence on Health and Cost Outcomes
Another study examining CAPABLE implementation found that participants experienced:

	Improved daily functioning
	Lower depression scores
	Fewer avoidable hospitalizations and readmissions

These findings suggest CAPABLE can improve health outcomes while potentially lowering healthcare costs.

National Momentum for Aging in Place
Across the United States, interest in aging-in-place programs continues to grow.

Recent news highlights include:

A WellSpan-supported CAPABLE initiative helping older adults in Pennsylvania remain safely in their homes.
A proposal in New York's 2026 State of the State to introduce a CAPABLE program to support older residents.

These developments reflect increasing recognition that supporting independence at home benefits individuals, families, and healthcare systems.

Recognizing the Professionals Who Make Aging in Place Possible
The coming months also highlight the professionals and public health efforts that support older adults:

Occupational Therapy Month (April) celebrates OTs who assess home environments and recommend modifications that improve safety and independence.
National Public Health Week (April 6–12) focuses on prevention and community health—principles central to CAPABLE's design.
Older Americans Month (May) honors the resilience and contributions of older adults nationwide.
National Nurses Week (May 6–12) recognizes the nurses who help participants manage chronic conditions and achieve personal health goals.

Together, these observances underscore the collaborative approach that makes CAPABLE successful.

Strengthening CAPABLE Programs for the Future
As CAPABLE programs expand, many organizations are focusing on strengthening implementation and sustainability through:

Cross-sector partnerships with housing providers and aging organizations
Workforce support for nurses, occupational therapists, and home modification specialists
Data collection and storytelling to demonstrate outcomes and impact

Investing in these areas ensures CAPABLE continues to deliver person-directed, evidence-based solutions that help older adults age safely at home.

Read the Full CAPABLE Newsletter
Explore the full newsletter to learn more about research, policy developments, upcoming events, and real stories from CAPABLE participants.

Read it here: https://mailchi.mp/jhu/capable-mar-may-2026-newsletter-17461135

From Gratitude to Giving: Community Fundraiser Supports Hospice Care at The Denver Hospice

When Compassion Inspires Giving
At The Denver Hospice, we have the privilege of walking alongside families during some of life's most meaningful and difficult moments. The care we provide is about more than managing symptoms—it's about offering comfort, dignity, and support to patients and the people who love them.

For one Denver-area husband, Jack Brosky, the care his wife received last year made a lasting impression.

After her time in hospice, he felt compelled to give back in a way that would help other families receive the same compassionate support.

Turning Gratitude Into Action
Earlier this year, he organized a Poker Run fundraiser, bringing together friends, fellow riders, and community members for a day of connection and purpose.

A Poker Run is a community event where participants travel between designated stops, collecting playing cards along the way. At the end of the route, the best poker hand wins—but the true goal is often something greater: supporting a cause that matters.

Through this event, he raised funds to support the mission of The Denver Hospice.

Recently, he presented his donation to our team, sharing that his motivation was simple—he wanted other families to have access to the same compassionate care his wife received.

Ensuring Hospice Care Is Available to All Who Need It
At times, families may face difficult circumstances when serious illness arrives. When that happens, hospice providers often step forward to ensure patients can receive care focused on comfort, dignity, and quality of life.

Community generosity helps make that possible.

Donations like this one allow The Denver Hospice to continue providing care and support to families across our region, ensuring that financial barriers do not stand in the way of compassionate end-of-life care.

The Power of Community
Stories like this are powerful reminders that hospice care extends far beyond medical services. It creates lasting connections between caregivers, patients, families, and the communities that surround them.

We are deeply grateful to Jack and to everyone who participated in the Poker Run. Their kindness helps ensure that more families can receive the support they need during one of life's most important journeys.

Together, our community continues to make compassionate care possible.

Recently, Tricia Ford, Chief Operating Officer – Hospice/CAPABLE, helped bring attention to the impactful work of the CAPABLE Program during a special breakfast conversation at the Jewish Association Serving the Aging Annual Conference 2026.

Hosted for staff and partners of Jewish Association Serving the Aging, the breakfast session introduced attendees to the CAPABLE model and highlighted how the missions of both organizations strongly align. CAPABLE focuses on helping older adults remain safe and independent in their homes by addressing functional needs through a holistic, interdisciplinary approach.

During the conversation, participants explored how the philosophies and services of JASA and CAPABLE complement one another. Both organizations prioritize empowering older adults, promoting functional independence, and supporting individuals so they can continue living safely in the place they call home.
Events like this create opportunities to share knowledge, spark collaboration, and strengthen the collective effort to support aging in place.

Thank you to Tricia for representing Care Synergy and helping expand awareness of the CAPABLE program and the difference it can make for older adults and their families.

Caregiving in Community: A Film and Conversation on Support, Strain & Strength

Caregiving is something that touches nearly every family. Whether supporting an aging parent, a spouse facing serious illness, or a loved one nearing the end of life, caregivers play an essential role in our communities. Their work is filled with compassion, resilience, and dedication—but it can also bring emotional, physical, and practical challenges.

To explore these realities together, Rocky Mountain PBS and Pikes Peak Hospice & Palliative Care invite community members to a special event that highlights the experiences of caregivers and the importance of support.

On Saturday, April 11, community members are invited to gather at the Colorado Springs Senior Center for "Caregiving in Community: A Film and Conversation on Support, Strain & Strength."

A Powerful Documentary Preview
The event will feature a special preview of the documentary Caregiving, presented by Rocky Mountain PBS through its Allies for Aging initiative. The film explores the realities of caregiving in America—highlighting the compassion, complexity, and challenges that caregivers experience every day.
Through powerful storytelling, the documentary brings attention to the often unseen work of caregiving while honoring the individuals who provide this vital support to family members and loved ones.

A Community Conversation
Following the film preview, attendees will have the opportunity to participate in a community conversation about caregiving. The discussion will focus on the themes of support, strain, and strength, exploring both the challenges caregivers face and the resources available to help.

As a co-host of the event, Pikes Peak Hospice & Palliative Care brings deep experience supporting patients and families through serious illness, grief, and end-of-life care. The organization's involvement underscores the importance of community connection, education, and compassionate care for those navigating the caregiving journey.

Why This Conversation Matters
Across the country, millions of people provide care for family members and friends. While caregiving can be deeply meaningful, it can also feel isolating or overwhelming at times. Events like this create space for community members to connect, share experiences, and learn more about available support.
By bringing caregivers, healthcare professionals, and community members together, the event aims to foster understanding, strengthen networks of support, and recognize the invaluable role caregivers play.

Event Details

Caregiving in Community: A Film and Conversation on Support, Strain & Strength

	Date: Saturday, April 11, 2026
	Time: 1:00–3:30 PM
	Location: Colorado Springs Senior Center
Presented by: Rocky Mountain PBS – Allies for Aging
Co-hosted by: Pikes Peak Hospice & Palliative Care
	Cost: Free with registration

Community members are encouraged to attend, learn, and take part in this important conversation about caregiving.

To learn more and reserve your spot, visit:
https://events.rmpbs.org/event/caregiving-in-community-a-film-and-conversation-on-support-strain-and-strength/e775130

How Partnerships Improve Hospice Care: Pathways and Superior Medical Equipment Supporting Patient Comfort at Home

At Pathways, caring for patients with serious illness requires more than compassionate clinical care — it also requires trusted partnerships that ensure patients have the equipment and resources they need to remain comfortable and supported at home.

That's why Pathways partners with Superior Medical Equipment, an ACHC-accredited provider of durable medical equipment (DME). Through this collaboration, Pathways patients receive high-quality medical equipment, reliable service, and timely support — all essential components of safe and comfortable care at home.

Durable medical equipment plays a critical role in hospice care. Hospital beds, mobility aids, oxygen equipment, and other specialized tools help patients remain comfortable, maintain dignity, and safely receive care in the environment they prefer most: home.
Just as important as the equipment itself are the people who deliver and support it.

Recently, a patient shared the following feedback about Will, a member of the Superior Medical Equipment team:

"Will was an amazing man! Perfect for this job! He was so caring, so polite, explained everything so thoroughly, and was professional all at the same time! The equipment is like new, updated, clean, and functioning well! I am so impressed and grateful for this company and the services!"

Stories like this highlight the impact that compassionate service and attention to detail can have on patients and families navigating a difficult time.

Pathways and Superior Medical Equipment are both affiliates of Care Synergy, a network of mission-driven, values-based, not-for-profit healthcare organizations dedicated to caring for individuals with serious illness — often older adults with complex medical needs — by helping them remain at home, stay out of the hospital, and maximize their comfort and quality of life.

By working together, organizations across the Care Synergy network are able to deliver coordinated, compassionate care that supports patients and families when they need it most.

And thanks to dedicated professionals like Will, that mission is carried forward every day.

This coming April is National Volunteer Month, a time to recognize the people who strengthen communities through service. At Pathways, volunteers play an essential role in supporting individuals and families navigating serious illness, caregiving, and grief.

The Impact of Hospice and Grief Support Volunteers

Volunteers help expand Pathways' ability to provide compassionate care across our community. Their contributions take many forms, including:

Providing companionship to individuals facing serious illness
	Supporting families and caregivers
	Assisting with grief support programs
Helping with community events and outreach
	Offering administrative and behind-the-scenes support

While their roles vary, the impact is the same: ensuring people feel supported and connected during some of life's most challenging moments.

Why Volunteers Matter in End-of-Life and Grief Care

In hospice and grief support, presence matters. Volunteers bring time, attention, and compassion that can make a meaningful difference for patients and families. Their support helps ensure that no one in our community has to face illness, loss, or grief alone.

Honoring Our Volunteers

During National Volunteer Month, Pathways is proud to celebrate the dedicated individuals who give their time and heart to this work. Their generosity allows us to reach more people and provide the compassionate care our community deserves.

Learn more about the volunteers who support Pathways in our latest newsletter: https://conta.cc/4rnI9Tc

If you're interested in giving back, volunteering is one of the most meaningful ways to support individuals and families in your community.
Honoring Loved Ones at Our Candlelight Remembrance Ceremony

Last night, The Denver Hospice welcomed families and community members to a Candlelight Remembrance Ceremony, a meaningful evening dedicated to honoring and remembering loved ones who have passed.

Grief is deeply personal, yet it is also something that connects us. Our Candlelight Remembrance Ceremonies create a space where individuals and families can come together to reflect, share memories, and find comfort in community.

During the ceremony, the names of loved ones were read aloud, offering a moment to pause and recognize the lives that continue to live on in the hearts of those who knew them. Families also had the opportunity to share photos, creating a visual tribute to the people who shaped their lives.

The evening concluded with a candlelight ritual. As each candle was lit, the room filled with a quiet glow, symbolizing the enduring presence of those we remember. Each flame represented love that continues beyond loss and memories that remain a guiding light.

Moments like these remind us that remembrance can be both tender and healing. By gathering together, we acknowledge the weight of grief while also celebrating the lives that touched our own.

At The Denver Hospice, we are honored to provide spaces where families can reflect, remember, and connect with others who understand the journey of loss. These ceremonies are a reminder that while grief can feel isolating, no one has to walk through it alone.

To all who joined us, thank you for allowing us to share this evening of remembrance with you.

Finding Expression Through Art: A Creative Approach to Grief Support

On a snowy Friday afternoon, a small group gathered at The Denver Hospice with paint, paper, and open hearts. They were there for a session of The Denver Hospice's Creative Art Group, a six-week grief support program designed for adults who have lost a loved one.

Grief can be complex and deeply personal. While conversation and traditional support groups are helpful for many people, sometimes emotions are difficult to express with words alone. Creative expression offers another pathway. Through art, participants can explore memories, emotions, and experiences that may feel too heavy or complicated to describe.

The Creative Art Group invites participants to engage in activities such as painting, drawing, and collage. These creative exercises provide a gentle way to reflect on loss, honor the memory of loved ones, and acknowledge the many emotions that accompany grief. The focus is not on artistic ability or producing a perfect piece of art. Instead, the emphasis is on the process itself.

Art allows people to slow down and connect with their feelings in a different way. A color choice, a shape, or an image can represent something meaningful when words fall short. For many participants, the act of creating becomes both reflective and therapeutic.

Another important aspect of the group is community. Participants share the experience with others who understand the weight of loss. In a supportive environment, creativity becomes a bridge that helps people connect, listen, and learn from one another.

No previous art experience or talent is necessary to participate. The group is open to anyone who has experienced the loss of a loved one and is interested in exploring grief through creativity.

With a winter storm in the background, the room filled with conversation, reflection, and quiet moments of creativity. Each piece of art told a different story, and together they represented the many ways people remember, honor, and carry forward the lives of those they love.

What If We Just Kept Driving?

For one family connected to Pathways Hospice, Baja is more than a destination. It's a place woven into their story.

For nearly 15 years, they traveled there together at least once a year — creating memories in the desert landscape, surrounded by cacti, sunshine, and time together as a family. It became one of their favorite places, especially for a father who cherished those trips.

Recently, as this family made an emotional drive to Pathways' Inpatient Care Center, those memories came rushing back.

During the ride, his wife asked a simple but heartfelt question: "What if we just kept driving south? Do you think we could make it to Baja?"

He answered with a bit of humor and honesty.

"We'd need a truckload of oxygen."

Their daughter, an artist, later turned that moment into a drawing. In the illustration, a truck carries a towering stack of oxygen tanks as it heads toward Baja through a desert landscape. The tanks represent exactly what her father joked about — the oxygen it would take to make the trip.

But the image represents much more than that.

It captures the crossroads families often face at the end of life — the longing to keep going, the memories that shape us, and the love that surrounds difficult decisions. The road to Baja becomes a symbol of possibility, memory, and the life that came before the moment of saying goodbye.

For the Pathways team, the artwork is a powerful reminder of the humanity within hospice care. Every patient brings a lifetime of stories, places they love, and moments that define them.

Hospice is about honoring those stories. It's about making space for families to reflect, to remember, and to choose the path that feels right for them.

We are deeply grateful to this family for sharing such a personal piece of their journey. Through one drawing and one conversation in a car, they captured something universal — the deep love that carries families through life's hardest moments.

Saving Healthcare Costs in the Real-World: Implementation of CAPABLE in Population-Based Care

The need for evidence-based services that help older adults thrive in the community aligns with the economic need to lower costs while achieving better outcomes. Some hospitals with global budgets have started paying for community-based care to decrease hospitalizations.

In a study titled "Saving Healthcare Costs in the Real-World: Implementation of CAPABLE in Population-Based Care," researchers evaluated health, utilization, and cost outcomes associated with CAPABLE, a time-limited intervention to improve daily function, among 205 participants. While not achieving statistical significance, participants experienced fewer avoidable hospitals stays and readmissions. Participants had significant improvements in functional outcomes and decreased depression scores.

Implementing CAPABLE has potential to yield cost-saving and health improving benefits for hospitals with global budgets and insurers by reducing preventable hospitalizations.

View the study here.

Earlier this year, Pikes Peak Hospice & Palliative Care (PPHPC) asked its community for help replenishing our supply of blankets for veteran patients in our care. In early January, they launched a blanket drive requesting new red, white and blue blankets and set an ambitious goal of collecting 100 by March 2.

The response exceeded their hopes. With help from coverage in The Gazette, Fox 21, and the power of social media, word spread quickly throughout the community. Knitters, quilters, crocheters, and generous supporters who donated through our Amazon Wish List stepped forward to help.

Thanks to that generosity, PPHPC has received more than 150 blankets and counting.
Each blanket represents comfort, appreciation, and recognition for the veterans we serve. But the drive also led to another wonderful surprise.

When Mrs. Cooper's kindergarten class at New Summit Charter Academy heard about the effort, they turned it into a kindness project. The students wrote and colored letters to thank veterans for their service. Their messages, filled with drawings and heartfelt gratitude, have been shared with veteran patients and have brought many smiles.

We are deeply grateful to everyone who contributed blankets, shared our message, or helped spread the word. And a special thank you to Mrs. Cooper's class for reminding us that kindness can come from even the smallest hands.

Tommy’s Express Car Wash Helps Pikes Peak Hospice Care Teams

Pikes Peak Hospice & Palliative Care (PPHPC) care teams travel daily to every corner of their service region to meet the needs of patients and families, providing expert and compassionate care wherever patients call home. In fact, last year PPHPC care team members – nurses, social workers, CNAs, physicians and chaplains – drove 418,854 miles to serve patients. Their cars are their mobile office, where they document patient visits, transport supplies, consult by phone with physicians, and often where they grab coffee or a bite of lunch.

When the folks at Tommy's Express Car Wash learned about how much PPHPC care teams are on the road each day, they stepped up – big time. Becoming annual sponsors, Tommy's provides free exterior and interior car washes to PPHPC mobile care team members, which, in turn, supports care teams in providing the best service to patients, 24-7, 365 days a year. This generous sponsorship not only ensures mobile employees have clean cars on the outside, but also on the interior, thanks to Tommy's commitment to 15-minutes-or-less interior cleaning services.

"When we learned of the powerful work of Pikes Peak Hospice, we knew we could help," said Megan Poe, co-owner of three Tommy's franchises in Colorado Springs (and a fourth on the way). "Treating the nurses and others on the hospice care teams to a clean car, inside and out, gives them a better work environment and lets them know we value the love and care they provide all year to so many patients and families in our community. It's a real honor to support the mission of Pikes Peak Hospice this way."

Tommy's has locations at 4089 Tutt Blvd., 4365 Venetucci Blvd., and at 1540 Garden of the Gods Road, which also boasts their total interior cleaning service – the only one of its kind in the city.

In early January, Pikes Peak Hospice & Palliative Care (PPHPC) asked their community to fill our depleted supplies by donating new red, white and blue blankets to give to veteran patients in our care. We set a wildly ambitious goal of 100 blankets by March 2. Word spread quickly through The Gazette, Fox 21, and social media. And, through the generosity of scores of knitters, quilters, crocheters and folks who gave through an Amazon Wish List, they've received over 150 blankets (and counting)!

Thank you! This incredible outpouring demonstrates that they live in a loving community that appreciates our veterans and understands the vital importance of compassion and dignity near end of life. In 2025, PPHPC served 458 veterans and performed 125 pinning ceremonies; these gifts of a blanket are cherished by patients and their families.

As one woman shared in response to a Facebook post: "It means so much more than anyone would think to receive a blanket at this time and need in their lives. My mom received a lap quilt that will be passed to her granddaughter who was born days before she passed. Now her granddaughter will have a closeness to someone she never met, that no one else will ever have."

To all those who contributed, THANK YOU. And to those still creating blankets, PPHPC welcomes future contributions of blankets. Many have asked about other ways to support our veteran care. Monetary donations can be made here, and if you wish your gift to be designated to our veterans program, simply indicate in the comments field. PPHPC also seeks veteran volunteers to provide companionship visits with our veteran patients; for more on volunteering go here.

Pictured is Pikes Peak Hospice Veterans Liaison Sonya Bergeron giving a pin and handmade blanket to 100-year-old Russell Brehm, an Army veteran who served in World War II, the Korean War and the Vietnam War.

Care Synergy In The News 2.26.26

Yesterday morning, Pathways team members Natalie Petersen and Ryan Bluth joined Brian and Tanner on Good Morning NOCO to discuss one of the most important — and often misunderstood — topics in healthcare: end-of-life care.

During the conversation, they explained when families should consider reaching out for hospice support, what services are available, and why timing matters. One key takeaway: every person and every situation is unique. Hospice is not a one-size-fits-all service, and individuals have the right to make informed choices about their care.

Many people are surprised to learn that hospice support can begin earlier than they expect and that it focuses on comfort, dignity, and quality of life.

At Pathways, our role is to guide patients and families through these difficult moments with clarity, compassion, and expertise.

Listen to the full interview here:
https://1310kfka.com/podcasts/noco-now/02/good-morning-noco-february-25-2026-crp-4x4-vip-guest-natalie-peterson-and-ryan-bluth-with-pathways-hospice/

Front Desk, Full Hearts: The Friendship That Greets You at Pathways

At Pathways, volunteerism is rooted in compassion — but sometimes, it also blossoms into something even more beautiful: friendship.

For Mary and Tonia, what began as a desire to give back has grown into a partnership that has quietly supported patients, families, staff — and each other — for years.

Mary's Story: Hospice Is in Her Roots
Mary is a retired nurse whose connection to hospice runs deep. Her mother was the first hospice nurse in Fort Collins — long before hospice was widely understood or embraced.

Years later, when both of Mary's parents became patients at Pathways, she experienced firsthand the same compassionate care her mother had once helped pioneer. After recovering from back surgery and moving to Fort Collins, Mary began volunteering.

That was 32 years ago.

Today, Mary assists with reception and is a member of the Board of Directors. Her steady presence, calm demeanor, and deep institutional knowledge make her a gift to everyone who walks through the doors.

Tonia's Story: Gratitude Turned into Service
Tonia's introduction to Pathways also came through her parents. Both her mother and father received care here, and the experience left a lasting impact.
After they passed, Tonia found herself looking for meaningful ways to give back. With a background in school nutrition services, she understood service and organization — but what she really carried with her was gratitude.

She began volunteering 11 years ago and never looked back.

A Wednesday Tradition
For years — they estimate five or six, though neither can quite remember — Mary and Tonia have shared Wednesday front desk shifts.

But to call them simply "co-volunteers" would miss the heart of their story.

They are true partners.

Tonia comes in early to get Mary set up for the day — pulling out her favorite music and making sure her beloved Twizzlers are ready. When asked why she does it, Tonia's answer is simple:

"Because Mary is amazing."

Mary, in turn, always makes sure Tonia has lunch. It's a quiet, mutual rhythm of care — the same kind of care they offer to patients and families, extended naturally to each other.

Their partnership isn't just about answering phones or greeting visitors. It's built on a shared desire to support not only those receiving care, but the staff who provide it.

They joke that they share both a "common love for Pathways" and a "common back pain." Beneath the humor lies something deeper — shared experiences caring for their own parents, navigating their own health challenges, and choosing to continue showing up anyway.

Friendship at the Front Desk
Volunteering together has made their experience more joyful. What could feel like a shift feels instead like time spent with a dear friend.

They love the people.
They love the passion.
And yes — they love the free cookies.

But most of all, they love being part of something meaningful.

Their story is a reminder that volunteer appreciation isn't just about recognizing hours served. It's about celebrating the relationships built, the lives touched, and the quiet ways volunteers care for one another behind the scenes.

Mary and Tonia embody what makes Pathways special: compassion that extends in every direction — to patients, to families, to staff, and to fellow volunteers.

If you've ever considered volunteering, know this: you may come to serve.

You may stay because it matters.

And if you're lucky, you might just find a partnership like Mary and Tonia's along the way.

Pathways offers a variety of ways to become involved in clinical and non-clinical roles within our community. The We Honor Veterans Volunteer Program gives those interested special ways to honor the service of our veterans at the end of life.
Colorado PACE Participant Spotlight: Barb Curtis

Barb Curtis may be a participant at Colorado PACE, but she's also become one of our most enthusiastic ambassadors.

Barb began receiving services from Colorado PACE in October after growing frustrated with the support she was receiving from a previous provider. Since then, her experience has been transformative. "Colorado PACE has been a godsend," she says. "I just can't say enough good things about it. Every person here cares about you, listens to you, and gives you the opportunity to make your own choices."

Many of Barb's choices at Colorado PACE center around her love of the creative arts. She is especially skilled at diamond art, a detailed craft that uses tiny resin "diamonds" placed on a sticky canvas to create colorful, shimmering designs. Several of Barb's pieces are proudly displayed throughout the Colorado PACE Center, including a majestic eagle she created with great care and patience.

Barb credits Colorado PACE with improving her life in meaningful ways. "I am not the same person I was before," she shares. "Colorado PACE has helped me come out of my shell." Anyone who knows Barb can attest to that—she's quick to share her positive experience with friends, neighbors, and even strangers, often encouraging others to explore Colorado PACE for themselves.

Barb attends the Colorado PACE Center five days a week and says simply, "When I'm not here, I miss it." We miss you when you're not here too, Barb—and we're grateful to have you as part of the Colorado PACE family and as a passionate advocate for the work we do.

Care Synergy In The News 2.19.26

For more than 22 years, Pat DesJardin quietly and faithfully served as a volunteer care companion with Pathways Hospice.

Recently featured in Fifty & Better Magazine (check it out here on page 38: https://issuu.com/rmparent/docs/fifty_better_spring), Pat reflected on his long journey in hospice volunteering—and the lessons it taught him.

After moving to Loveland in 1997 to be closer to family, Pat searched for a meaningful way to give back. A former high school biology, anatomy, and physiology teacher—and longtime coach—he had always been a nurturer. When he learned about hospice volunteering, he completed Pathways' training and began visiting patients two hours each week.

Over the years, Pat companioned patients in nursing homes, assisted living communities, and private residences. He read aloud. He played games. He shared meals. He pushed one woman's wheelchair through a sculpture park because she loved art. He played Farkle with a man living with Parkinson's. Often, he simply listened.

"I learned to be a good listener by shutting my mouth," Pat says. "They didn't want to hear about me. They needed to talk."

Some patients were in his life for weeks; others for years. One woman in her 90s remained his companion for six years. With each relationship, Pat offered presence, steadiness, and compassion.

As he recently retired at age 86, he reflected on the emotional investment hospice work requires. "As I became more emotionally involved with my patients, and realized my abilities were less than they were in my 60s, it became apparent it was time to retire," he said. "Volunteering with hospice has been fun and sometimes sad."

When asked about his favorite part of volunteering, his answer was simple: the people.

"Everyone is different, but all of them are my heroes. They are so strong and appreciative of the little I do for them, even though they do so much for me."

Pat encourages others to consider hospice service, acknowledging it isn't easy—but deeply rewarding. His advice: "Be kind; be ready. Be strong, friendly, prepared, compassionate—and above all, be yourself."

We are profoundly grateful for Pat's decades of service. He may say he gained more than he gave, but the families and patients he walked beside would likely say the opposite.

"I'll cherish the many memories of those who left footprints on my heart," Pat shared.

And we will cherish the footprints he has left on ours.

We're thankful to FOX21 News for sharing our recent call for patriotic blanket donations supporting veterans in the care of Pikes Peak Hospice & Palliative Care.

At Pikes Peak Hospice & Palliative Care, honoring veterans is woven into the fabric of what we do. Through special recognition ceremonies and personalized acknowledgments, we strive to ensure every veteran feels seen, respected, and appreciated.

Our current initiative focuses on collecting new red, white, and blue blankets to present to veteran patients receiving hospice services. These blankets are more than a source of warmth — they are a tangible expression of gratitude from the community they served.

We are working toward a goal of 100 donated blankets and invite individuals, families, schools, and community groups to participate. Blankets may be handmade or newly purchased, as long as they reflect patriotic colors.

To learn more about the initiative, watch the full FOX21 story here:
https://www.fox21news.com/news/local/pikes-peak-hospice-looking-for-patriotic-blankets-for-veterans/

We are continually inspired by the generosity of our community.
CAPABLE Client Spotlight: Paul Pickrem

As a journalist, Paul Pickrem has spent much of his life learning how to listen. Over decades of reporting, he has honed a professional instinct for hearing not only what people say, but what they leave unsaid—and what lies beneath the surface. When Paul participated in the CAPABLE program in Nova Scotia, he immediately recognized that same attentiveness at work. What stood out to him was not simply the home modifications or equipment, but how the CAPABLE team listened carefully and responded with purpose.

Paul, who will turn 72 this year, lives on a limited income. Increasing mobility challenges, severe arthritis, and the cumulative effects of aging shape his daily life, as they do for many older adults. Tasks that once felt routine—cutting an onion, squeezing an eye-drop bottle, doing laundry—gradually became difficult or impossible. While medical care addressed diagnoses and treatment plans, it often left the practical realities of day-to-day living unresolved.

CAPABLE stepped directly into that gap. From the beginning, Paul noticed how different the team's approach felt. "They didn't come in and tell me what I needed," he explains. "They asked me." As someone attuned to power dynamics, that distinction mattered. Week after week, the occupational therapist, nurse, and carpenter returned to his home, asked thoughtful questions, and adjusted plans based on Paul's priorities. "CAPABLE clients aren't passive," he says. "You're part of the process. It wasn't something done to me—it was done with me."

That attentiveness translated into action. What impressed Paul most was the team's follow-through. "They did everything they said they would do," he notes. In a healthcare landscape where promises can feel abstract or delayed, that reliability stood out. It gave Paul confidence that the changes underway would not only address his immediate needs but also support him as those needs evolved.

The adaptations ranged from modest to transformative. Bathroom safety improvements included a shower bench with grab bars. Kitchen adjustments restored his ability to wash dishes and prepare food safely. Grabbers, adaptive utensils, compression socks, and tools to help put them on reduced strain on joints affected by arthritis and tremors. Shelving and decluttering strategies made movement through his home safer and more efficient. These were not conveniences for Paul; they were essential supports that allowed him to remain independent.

Paul received this care through Victorian Order of Nurses (VON), a non-profit organization that has been providing home and community care in Canada for over a century. The organization has a mission to help people live in their homes and communities by delivering the caring support that they and their families need. VON delivers the CAPABLE program in partnership with Johns Hopkins and Nova Scotia's Department of Seniors and Long-term Care.

As someone who writes about marginalized communities and aging populations, Paul understands the broader context surrounding programs like CAPABLE. Nova Scotia has one of the oldest populations in Canada, and many seniors struggle quietly, unsure what help exists or how to access it. "Most people don't even know the CAPABLE program exists," he says. When he talks about his experience, he frames it not as charity, but as opportunity—an opportunity to be heard and taken seriously. "I tell people to find out more and see if it's right for them."

Looking back, Paul describes CAPABLE as rare and forward-thinking—a program that treats older adults as equal members of the care team. "I'm on the team," he says. "That is a big deal." As a journalist, Paul knows that careful listening uncovers the story beneath the surface. He sees that same principle at the heart of CAPABLE: the team listens deeply, identifies what truly matters in a person's daily life, and responds in ways that allow that story ending to change for the better.

Colorado PACE Participant Spotlight: Josie Lobato

To describe Josie Lobato's life as active would be a serious understatement. Over the years, she has raised eight children, worked at the Colorado Historical Society, written books, and created stunning colcha embroidery—traditional Southwestern folk art rooted in the Hispanic and Indigenous cultures of northern New Mexico and southern Colorado. She was even awarded a National Heritage Fellowship medal from the National Endowment for the Arts in 2019!

Her work, like her life, is rich with color, history, and purpose.

As Josie approaches her 90th birthday, age and health challenges have made it harder for her to live at the pace she once did. "It's important to keep yourself entertained," she says, reflecting on the frustration of slowing down.

Through Colorado PACE, Josie has found a way to remain active, creative, and connected. She visits the Colorado PACE Center three times a week, and those days are often the highlight of her schedule. "I would never have been happy in a nursing home," she shares. "This has been a blessing to me."

Perhaps most meaningful of all, Colorado PACE has allowed Josie to remain independent in her own home—where she can continue caring for her beloved cat, Mika. As Josie puts it best, "You really can't find a better place for services."

We are grateful to Josie for trusting Colorado PACE with her care and honored to share her inspiring story.

Welcoming Dr. Julie Bemski as Medical Director of The Denver Hospice

At The Denver Hospice, compassionate care begins with thoughtful leadership. We are pleased to welcome Julienne (Julie) Bemski, DO, who joins our organization as Medical Director beginning January 20, 2026.

Dr. Bemski's path to hospice and palliative care reflects a deep respect for the whole person — a philosophy shaped by years of full-spectrum family medicine practice and osteopathic training. After caring for patients across all stages of life, she felt drawn to focus her work on supporting individuals and families navigating serious illness and complex medical decisions.

"Julie brings both clinical excellence and a deeply human approach to care," said Ben Marcantonio, President of The Denver Hospice. "She understands that hospice and palliative care are about more than medicine — they're about listening, dignity, and walking alongside people when it matters most."

Currently completing a Community Fellowship in Hospice and Palliative Care at the University of Colorado Anschutz Medical Campus, Dr. Bemski brings the most current evidence-based practices to her role while honoring the values that have guided hospice care for decades.

"I am incredibly grateful for the opportunity to join The Denver Hospice," said Dr. Bemski. "The mission here resonates deeply with me. Providing comfort, clarity, and compassionate care to patients and families during serious illness is a profound responsibility, and I'm honored to be part of a team so dedicated to that purpose."

Prior to joining TDH, Dr. Bemski served as Associate Program Director for the Swedish Family Medicine Residency, where she supervised residents and medical students, led curriculum development, and created initiatives focused on osteopathic medicine, physician wellness, and scholarly activity.

"Julie's leadership experience and commitment to education will strengthen not only our clinical programs, but also the support we provide to physicians and care teams," Marcantonio said. "She is a wonderful addition to our organization."

As Medical Director, Dr. Bemski will collaborate closely with interdisciplinary teams to guide clinical practice, ensure regulatory excellence, and support the delivery of compassionate, high-quality hospice and palliative care throughout our community.

Outside of work, she enjoys hiking Colorado's trails with her father and dog and spending time with her husband and young son.

Reaching for the Moon: Jack’s Legacy in the Apollo Missions

Pictured: Jack Burman (far left) and the team that helped put humans on the moon, and Jack's Commemorative Certificate for his work on the Apollo missions

Meet Jack, an engineer who helped make history. After deciding on a career change while living in Baton Rouge, he packed up, moved to Boulder, and went on to work on several Apollo missions, including Apollo 13 and Apollo 14.

Jack was part of the team responsible for the design and implementation of the spacecraft's oxygen tank system. Students of history, and fans of the movie, know that during the Apollo 13 mission one of the spacecraft's oxygen tanks exploded because of damaged wiring insulation. The sudden loss of oxygen and power forced the crew to abort their lunar landing and use the Lunar Module as a lifeboat to make the long journey home.

Although Jack's team was not responsible for the wiring insulation itself, he was tasked with investigating exactly what had gone wrong. Carefully examining the detailed records kept by every company involved in the design and installation of the tanks, Jack was able to identify the precise moment and location where the faulty insulation had been created.

Although Jack received accolades for his work on other Apollo missions, including a commemorative certificate for the part he played in Apollo 14, he views Apollo 13 as his most remarkable success. The dedication of so many people working together to uncover the cause of the faulty insulation and to ensure that such a problem would never happen again still fills him with pride and awe 55 years later.

The experience also reinforced two of Jack's lifelong beliefs: think everything through carefully, and never overlook the small details.

Jack's stories offer a fascinating glimpse into one of humankind's greatest achievements. We at The Denver Hospice are honored that he shared them with us and encourage everyone reading this story to think of Jack's remarkable legacy and the part he played in carrying humanity's dreams beyond our own world each time you look up and see the Moon shining above.

Emily Petersen, Executive Director of PACE Programs at Care Synergy, was recently interviewed by Denver's 7 News about rising demand — and inadequate funding — for programs like home-delivered meals, transportation, and in-home care for at-risk seniors in our state. Emily's thoughtful and poignant comments were especially meaningful at a time when state lawmakers grapple with some of the biggest problems facing the fastest growing demographic in our state, as Colorado's population of older adults is projected to surge nearly 30% by 2035.

Check out the full story here.

Care Synergy is deeply honored to be selected to manage the Community Aging in Place – Advancing Better Living for Elders (CAPABLE) national program. CAPABLE was developed by the Johns Hopkins University School of Nursing to provide older adults with resources to remain in their homes longer, achieve better health outcomes, and decrease medical costs.

What makes this even more exciting is the Colorado Visiting Nurses Association (CVNA) is the first community-based home health organization in the U.S. to implement CAPABLE. The handoff was official when Dr. Sarah Szanton the Dean of Johns Hopkins University School of Nursing visited the Amy Davis Center and awarded the responsibility of managing the CAPABLE National Center to Tricia Ford, Sr VP of Operations at Care Synergy.

In addition to overseeing daily operations, Tricia will be responsible for managing Amanda Goodenow, OT Strategic Partnership Coordinator; Colleen Morton, RN CAPABLE Manager; Gina Machado, RN existing sites, expansion of newly licensed sites, discovery efforts to identify funding sources, and administrative support.

There was no better way to celebrate CAPABLES's first anniversary when the the Care Synergy team met one of its first milestones of enrolling 100 people into the CAPABLE program.

Special thanks to Tricia Ford, SVP of Operations at Care Synergy for initiating and managing the relationship with Johns Hopkins as well as heading up the implementation of this strategically important program. In addition, a big thank you to Care Synergy and CVNA employees for taking on this responsibility.
