Care Synergy and CAPABLE National Center Featured in Forbes Article on Patient Safety
Care Synergy and the CAPABLE National Center are proud to be featured in a new Forbes article exploring how practical, community-informed solutions can help improve patient safety and prevent avoidable harm.
Published October 26, 2026, “Patient Safety Requires Practical, Community-Informed Solutions” highlights innovative approaches to improving safety where people live, work and receive care. The article, written by Shantanu Agrawal, MD, MPhil, for Elevance Health, features CAPABLE as an example of how community-based programs can help address important patient safety challenges.
Helping Older Adults Live Safely and Independently
CAPABLE, which stands for Community Aging in Place – Advancing Better Living for Elders, is a person-directed program designed to help older adults remain safely and independently in their homes.
For many older adults, maintaining independence means navigating everyday challenges that can increase the risk of falls or make daily activities more difficult. CAPABLE takes a personalized approach, recognizing that improving safety involves more than addressing medical needs alone. The home environment, mobility, daily routines and each person's individual goals all matter.
The program brings together occupational therapists, registered nurses and trained home-care workers to help participants identify barriers, develop practical solutions and make changes that support their safety and independence. Home modifications and individualized strategies help participants work toward the goals that matter most to them.
By putting older adults at the center of the process, CAPABLE demonstrates how a thoughtful, coordinated approach can help people continue living in the places they call home.
A Broader Approach to Patient Safety
Patient safety is not limited to hospitals and clinical settings. It also includes helping people manage their health at home, understand their care and reduce everyday risks that can lead to preventable injuries or complications.
The Forbes article highlights the Elevance Health Foundation's inaugural Patient Safety Prize, which recognizes community-informed solutions addressing three critical areas: health literacy, medication safety and fall prevention.
Care Synergy and the CAPABLE National Center were selected as an awardee for their work in fall prevention. The recognition provides an opportunity to further advance practical approaches that help older adults live safely and independently.
Advancing Safer, Healthier Futures
At Care Synergy, we believe meaningful improvements in care begin with understanding people, their circumstances and what matters most to them. CAPABLE reflects that commitment by combining clinical expertise with practical, individualized support to address challenges where they occur.
We are grateful to the Elevance Health Foundation for recognizing the potential of CAPABLE and for supporting community-informed innovation that can make a meaningful difference in people's lives.
