CAPABLE Shares Its Approach to Aging in Place at the Fall 2026 NAACOS Conference

Helping older adults live safely and independently at home is an important goal for health care organizations and communities. At the Fall 2026 NAACOS Conference, Amanda Goodenow, Director of the CAPABLE National Center, is sharing an innovative approach designed to help make that goal possible.

Amanda is attending the conference and hosting an informational booth to introduce health care leaders to CAPABLE (Community Aging in Place – Advancing Better Living for Elders), a person-centered program that helps older adults address challenges that can affect their ability to remain safely at home.

A team approach to aging in place

CAPABLE brings together an occupational therapist, a registered nurse and a handy worker to work alongside each participant. Rather than focusing solely on medical needs, the program considers the individual’s goals, daily activities and home environment.

Together, the team helps participants identify challenges and develop practical solutions that can improve safety, support everyday functioning and promote greater independence.

This approach recognizes that sometimes relatively simple changes to a home, combined with individualized health care support, can make a meaningful difference in an older adult’s ability to remain at home.

Connecting health care leaders with innovative solutions

The NAACOS conference provides an opportunity to share ideas and approaches with health care organizations focused on improving care and outcomes. By representing CAPABLE at the conference, Amanda is helping raise awareness of a model that addresses the relationship between an individual’s health, their home environment and their ability to age in place.

Care Synergy and the CAPABLE National Center are proud to advance approaches that help older adults pursue their goals, maintain independence and live safely in their communities.
