How Colorado PACE's Interdisciplinary Team Supports Every Participant
No two people have the same healthcare needs, which is why Colorado PACE believes care should never be one-size-fits-all.
Our Program of All-Inclusive Care for the Elderly (PACE) provides a comprehensive continuum of healthcare and supportive services that are customized for each participant. Rather than coordinating care across multiple providers and appointments, participants have an interdisciplinary team (IDT) working together on their behalf.
Based at the Colorado PACE Day Center, the IDT includes physicians, nurses, therapists, social workers, dietitians, personal care professionals, transportation staff, and other specialists who collaborate to develop and manage an individualized care plan. Every member of the team contributes their expertise to ensure participants receive the right care at the right time.
From primary medical care and rehabilitation services to medications, social engagement, nutrition, and transportation, every aspect of care is coordinated through one dedicated team. This collaborative approach helps reduce stress for participants and their families while improving communication and continuity of care.
