The 2025 Annual Report from Pathways offers a powerful reflection on a year centered around compassionate care, meaningful community connection, and unwavering support for patients and families across Northern Colorado.
Throughout 2025, Pathways was honored to care for more than 2,585 individuals through hospice and palliative care services while also providing grief and loss support to hundreds of community members. The report highlights the organization’s continued commitment to helping individuals and families experience more comfort, choice, dignity, and support throughout serious illness and grief.
The annual report also celebrates important milestones and achievements, including being named Best Hospice in Northern Colorado in the NOCO Style Readers’ Choice Awards and earning a four-star rating in the Medicare CAHPS® Hospice Survey. These recognitions reflect the dedication of the Pathways care teams, volunteers, and supporters who make compassionate, high-quality care possible every day.
Volunteerism remained at the heart of Pathways’ mission in 2025. The report highlights the incredible impact of 144 volunteers who contributed more than 12,400 hours of service supporting patients, families, grief programs, and community events. Stories shared throughout the report illustrate how compassion, companionship, and human connection continue to define the Pathways experience.
In addition to direct care services, Pathways strengthened community engagement through educational programs, advance care planning events, memorial services, and participation in initiatives such as the Walk to End Alzheimer’s. Fundraising events like A Vintage Affair also helped provide financial assistance and comfort resources for patients and families in need.
The report reflects not only the impact of Pathways’ services, but also the generosity and partnership of the Northern Colorado community. Every volunteer, donor, staff member, and supporter plays a role in ensuring individuals facing serious illness and grief receive compassionate care and meaningful support.
